Registration Packet

If you received word that your child has been enrolled at Peachtree Hope Charter School please call our School Registration Office at 404-622-2727 to schedule a time for your child's diagnostic testing and complete the enrollment application.

Please ensure the Registration Packet, along with the following documents are brought to our Registration Office at: 1807 Memorial Drive SE, Decatur, GA 30317

Documents needed:

1. Birth Certificate
2. Social Security Card
3. Immunization (Shot Records)
4. Physical (Ear, Eye, Dental)
5. Latest Report Card (1^st - 5^th grade students only)
6. CRCT scores
7. Proof of Residency
   1. a. Identification
   2. b. Utility Bill and/or Lease Agreement
